There's a good chance that the hand is either contaminated or the hand spring is broken. Often times if the spring is broken the cylinder will advance if the action is worked with the muzzle aimed toward the he floor. Gravity doing its thing to help.

The cylinder will turn if I use the single action motion, but not turn in a double action mode.
Okay, now I understand. I'm going to let some of the more gunsmith minded folks answer you. I have some ideas, but I'm not in the same league as some of the experts here, but I'm sure they can point you in the right direction, good luck!
